…lt;Tab/> There needs to be an understanding that the continuous passing of the blame is not helpful. The clinicians can not blame the pharmacist and vise versa. They need to stand side by and together hand in hand the problem will be resolved. Also individuals need to take responsibility and once the meaning of care comes back to the system the problem would be resolved as well. <Tab/>
